Your mission

• Provide counseling, examination, and treatment services, as well as nursing care and basic care for high-quality management of clients
• Actively contribute to the clients' maintenance at home through prevention and health promotion measures while taking into account individual needs with personalized care and by promoting the autonomy of clients
• Participate in the development of the clients' therapeutic project within the interdisciplinary team by gathering and transmitting your observations and proposals
• Participate in development projects and working groups around the continuous improvement of services in the field of home help and care
• Actively collaborate in the activities of interdisciplinary teams with your own role allowing you to value your nursing expertise as a full stakeholder in the health system

Your profile and your skills

• Bachelor in nursing or a title deemed equivalent (foreign diploma recognized by the CRS)
• Experience in the medico-social field is an asset and a marked interest in community health
• Ability to collaborate in a network and multidisciplinary manner
• Agility, sense of responsibility, priorities, and professional ethics
• Mastery of French orally and in writing
• Good ease with IT tools and mobile telephony
• Driver's license and vehicle indispensable
