This institution, consisting of a little more than 300 employees, is orienting the deployment of its strategic plan towards efficient and recognized integrated care. In this context, the RSBJ is looking for a Head of Integrated Care Development and Partnerships - 80 % Fixed-term contract 12 months: The position is likely to be extended, or even made permanent depending on future developments. It ensures the full management of projects related to the deployment of integrated care within the RSBJ. This position is part of a transformation phase of the RSBJ (partnerships, care pathways, governance) and aims to support the Management in these evolutions; its scope is expected to evolve, with stabilized missions intended to be taken over by internal management. It is financially supported by the Direction Générale de la Santé, and ensures the deployment of the cantonal project for the creation of a Pôle Santé on the Balcon du Jura. It also fits into the implementation of our strategy centered on integrated care. The Head of Integrated Care Development and Partnerships works in direct collaboration with the Conseil de direction. He/she is a permanent invitee to the weekly meetings of the Conseil de direction. Your mission: Under mandate from the executive committee: * Ensure strategic steering and support for governance on the subject of integrated care * Support the Executive Committee and the Conseil de direction in the analysis and decision-making on the structuring evolutions of the RSBJ (partnerships, care pathways, institutional positioning) * Represent the RSBJ, under mandate, in discussions with institutional partners --- notably the reference hospital --- with a view to strengthening collaborations * Develop partnerships and the health-social network * Support the progress of the different phases of projects related to the development of integrated care * Coordinate actors involved in health trajectories on the Balcon du Jura * Pool and coordinate the different internal and external projects relating to integrated care * Coordinate and steer all institutional projects * Develop community health, prevention and health promotion Your profile: Training: * Holder of a HES level training in the health field * CAS, DAS or other training deemed equivalent in the field of change management, project management and/or process management Experience: * Minimum 5 years in the health field, including good experience in the community field * Management of large-scale projects * Network work and coordination of care * Process approach and according to lean management Relations: * Leadership * Sense of human relations, listening and ease in oral communication * Ease in working in an interdisciplinary team * Negotiation skills * Autonomy, spirit of initiative * Good stress management, energy and tenacity Knowledge: * Public health, federal health system and Vaud health system * In-depth knowledge of the hospital, housing, medico-social support structures and home care domains Specific skills: * Perfect mastery of the French language (written and oral) and strong writing skills * High skills in analysis and systemic vision * Mastery of management, planning and office tools We offer: * A rich and interesting position in a stimulating environment allowing you to be part of innovative and large-scale projects * Attractive working conditions (CCT San scales) Start date: immediately or to be agreed Information: Madame Corinne Morel, Director of the Care department Complete application file (CV, motivation letter, diplomas and work certificates) to be submitted until 31 October Réseau Santé Balcon du * Réseau Santé Balcon du Rosiers 29 1450 Sainte-Croix * [ ](< >) jpid3b78972jm jpit0939jm jpiy26jm
