In order to complete our teams, La Fondation Soins Lausanne is looking for immediately or to be agreed upon several:

Referent Nurse (M/F)

Activity rate between 80% and 100%

Fixed-term indefinite contract - daytime schedule and a few weekends

Your mission

• Accompany clients in their life project at home from a partnership perspective by giving meaning and coherence in close collaboration with the entourage and loved ones while developing bonds of trust
• Pilot the global and integrative care of clients while relying on internal staff and interdisciplinary teams and in good coordination with the institutions involved in the care network
• Elaborate the therapeutic project taking into account the complexity of care and develop it according to evolution
• Provide assessment, advice, examination and treatment services, as well as nursing care and basic care for quality care of clients
• Actively contribute to the sustainable home maintenance of clients through prevention and health promotion measures while taking into account individual needs with personalized care and by promoting the autonomy of clients
• Participate in development projects and working groups around the continuous improvement of services in the field of home aids and care

Your profile and your skills

• Bachelor in nursing or title deemed equivalent (foreign diploma recognized by the CRS)
• Experience in the medico-social field and a marked interest in community health
• Autonomy and decision-making skills
• Ability to collaborate in a network and multidisciplinarity
• Sense of responsibility and professional ethics
