Les Peupliers ensures 7j/7j at the 7 CMS of the Fondation Soins Lausanne day, evening and night replacements in order to guarantee the continuity of care for our clients.

In order to complete the team of the CMS des Peupliers, La Fondation Soins Lausanne is looking for the immediate future or to be agreed upon:

Coordinator Nurse (M/F)

Activity rate of 80%

Fixed indefinite duration contract

Your missions

• Identify, analyze the immediate needs of the client and establish the first intervention plan in order to optimize the continuity of care.
• Secure the return home of clients and coordinate care during the take-charge period.
• Provide care during the take-charge period.
• Ensure a telephone on-call service in the evenings, weekends and public holidays.
• Collaborate with the intervenors of the client network and respond to their requests.
• Intervene as needed in acute situations at home.

Your profile and your skills

• Bachelor in nursing or title deemed equivalent (foreign diploma recognized by the CRS)
• Experience in the medico-social field and in interdisciplinary collaboration
• Knowledge of the care networks of the canton of Vaud
• Sense of responsibility, priorities and autonomy
• Driver's license indispensable and vehicle desirable
