Day Nurse (HES) at 80%
We are looking for a day nurse to strengthen its health services. Your mission is to ensure the health responsiveness of several living groups. You guarantee the continuity and quality of nursing care, develop prevention and health promotion actions, and work in close interdisciplinary collaboration with socio-educational staff and network partners, in compliance with the institutional concept.
Mission :
Health responsiveness and continuity of care:

• Be the reference person regarding health for several living groups
• Ensure the continuity of preventive, curative, palliative and delegated nursing care
• Identify critical situations and respond to them in an adapted manner

Prevention and health promotion:

• Implement prevention, health promotion and screening actions
• Promote hygiene measures and behaviors favorable to health
• Inform and sensitize teams and the people welcomed to health issues

Interdisciplinary work:

• Collaborate closely with socio-educational staff in support
• Participate in interprofessional meetings and contribute to common reflections
• Work with external partners (doctors, hospital, CMS, etc.)

Follow-up and quality:

• Keep medical records and nursing writings up to date
• Develop health syntheses and care projects.

Profile:
Degree in HES or ES nursing, or title deemed equivalent
Strong interest in prevention, health promotion and community health
Experience in community health, an asset
Ease in interdisciplinary work with socio-educational teams
Sense of responsibility, autonomy and capacity for analysis
Human, collaborative and solution-oriented approach.
