Referent Nurse (M/F)

Activity rate between 80%

Fixed-term contract for an indefinite duration - daytime schedule and some weekends

Your mission

• Accompany clients in their life project at home from a partnership perspective by giving meaning and coherence in close collaboration with the entourage and loved ones while developing links of trust
• Lead the global and integrative care of clients while relying on internal interveners and interdisciplinary teams and in good coordination with the institutions involved in the care network
• Elaborate the therapeutic project taking into account the complexity of care and develop it according to the evolution
• Provide assessment, counseling, examination, and treatment services, as well as nursing care and basic care for quality management of clients
• Actively contribute to the sustainable home maintenance of clients through prevention and health promotion measures while taking individual needs into account with personalized care and by promoting the autonomy of clients
• Participate in development projects and working groups around the continuous improvement of services in the field of home help and care

Your profile and your skills

• Bachelor in nursing or a title deemed equivalent (foreign diploma recognized by the CRS)
• Experience in the medico-social field and a strong interest in community health
• Autonomy and decision-making skills
• Ability to collaborate in networks and multidisciplinarity
• Sense of responsibility and professional ethics
