As a member of the PCLD, you will be required to intervene throughout the Canton of Geneva for long-term replacements, varying from 6 to 15 months, within our branches. Your main mission will consist of ensuring the continuity of patient follow-up, acting as a referent in the management of their files and providing quality services. By joining the PCLD team, you will be able to: * Assume the role of referent in the assessment of patient needs, in accordance with the ethical principles of the nursing profession, within the home care team model. * Provide personalized nursing care, autonomous or on medical delegation, covering a full range of preventive, curative, educational, and palliative services, directly at the homes of patients of all ages. * Work in close collaboration with community care and health as well as multidisciplinary aids, delegating care according to established procedures. * Organize medico-social steps and ensure coordination with all stakeholders involved in a given patient's situation, by mobilizing the available resources of the primary and secondary network.
